To enhance the university's technical testing services, and in accordance with Article 5 of the "Regulations for the Establishment of the Center for Food and Cosmetic Safety Research," this "Fee Standard for Food Safety Laboratory Testing Items" is hereby established.
II. Sample Submission Guidelines
Before submitting samples, the client is requested to inquire about the testing items, fee standards, and required sample volume by visiting the Center's website or by telephone.
After confirming the testing items and completing the payment, the client must fill out the "Test Sample Submission Form" and deliver it along with the test samples to the Laboratory. For remote submissions, clients shall package and preserve the test samples appropriately (using refrigerated or frozen shipping if necessary) and send them to: "Food Safety Laboratory, Center for Food and Cosmetic Safety Research, Chang Gung University of Science and Technology, No. 261, Wenhua 1st Rd., Guishan Dist., Taoyuan City 33303, Taiwan".
The Laboratory does not provide services for testing mixed samples. All tests requested for a single sample shall be issued on a single test report. The Laboratory does not offer the service of splitting reports by individual test items.
The fee is calculated using the following formula:
Fee = (Number of Samples) × (List Price) × (Rush Service Multiplier) × (Discount Rate) × 1.05
The fee standards for all testing items are as detailed in Appendix 1. For expedited processing, the following multipliers apply:
Express Service: 1.4 times the list price.
Urgent Service: 1.96 times the list price.
IV. Payment Methods
Cash or Check: Obtain a payment slip from the Laboratory, pay the fee at the university's Cashier's Office, and receive a receipt from the Accounting Office. Return the second copy of the payment slip to the Laboratory to confirm payment. The sample submission process shall be deemed complete only upon confirmation of payment.
Bank Transfer: Transfer the payment to:
